Senior Services provides Medicaid (HCBS) reassessments for individuals receiving in-home services. Center for Human Services focuses on understanding each individual’s functional needs, living situation, supports, and ensuring services are aligned with those needs in a person-centered way.
About Senior Services
What We Do
During a reassessment, trained CHS employees meet with the individual, typically in their home, to review:
- Current medical and functional needs
- Ability to complete activities of daily living
- Safety and mobility concerns
- Informal supports from family or friends
- Whether current services are meeting the individual’s needs
While the reassessment itself is the primary service, staff often go beyond the assessment by helping connect individuals to additional resources, or referrals when unmet needs are identified. This may include guidance on community resources, benefits, or other programs that can help support independence.
Who Qualifies
Senior Services reassessments are provided to individuals who are enrolled in Missouri Medicaid and are receiving, or applying to continue receiving, Home and Community-Based Services. Eligibility is determined by the Missouri Department of Health and Senior Services (DHSS) based on medical, functional, and financial criteria.
Individuals do not self-refer directly to Senior Services for reassessments; referrals and assignments are coordinated through the state. To begin applying for in-home services, individuals or caregivers can contact Missouri Medicaid’s (HCBS) for guidance on next steps.

Senior Services reassessments are typically conducted in the individual’s home, allowing staff to better understand real-world needs, safety concerns, and living conditions. In some cases, reassessments may also take place in residential care facilities.
