ADA Notice
The Americans with Disabilities Act (ADA) was passed by Congress in 1990 and was amended by Congress in 2008. The ADA prohibits discrimination against people with disabilities in several areas, including employment, transportation, public accommodations, communications, and access to state and local government programs and services.
We at Center for Human Services pride ourselves on the quality and compassion of our staff, their outreach, and our mission. Denying anyone who qualifies access to our services would not only violate our principles, but it would also violate Federal law.
ADA Complaint Form
If you have a complaint about the accessibility of our transit system or service, or believe you have been discriminated against because of your disability, you can file a complaint. Please provide all facts and circumstances surrounding your issue or complaint so we can fully investigate the incident.
How to file an ADA complaint with Center for Human Services:
1 - Print, fully fill out and mail the form to Center for Human Services:

2 - We encourage that you file the complaint with us. However, you may file a complaint with the Missouri Department of Transportation or the Federal Transit Administration:

3 - Complaints must be filed within 180 days following the date of the alleged incident and should contain as much detailed information as possible.
4 - The form must be signed and dated, and include your contact information.
