Service Coordination for the Developmentally Disabled

Service Coordinators act as case managers that offer one-to-one support to those of all ages. Since its inception in 1996, the need for Service Coordination has grown, and Center for Human Services proudly provides this service to more than 4,700 people and families throughout Missouri.

The system may be complex to navigate, but working with a Service Coordinator is not. Service Coordination is a far-reaching and popular service that is free to you. For those who qualify, a Service Coordinator meets with you to establish a plan. With a solid plan in place, they direct the resources to turn the plan into action.

How We Do It

There are many resources and services available to those with physical and mental disabilities. So many, in fact, that knowing who to contact and asking the right questions can be confusing. Our Service Coordinators make navigating this complex environment their job. Their sole focus is on the client. They plan, coordinate, communicate, and evaluate these services to find the ones that are just right for you.

While our typical hours are from 8:00 am – 4:30 pm Mondays – Fridays, our Service Coordinators are flexible to meet your scheduling needs.

Who Qualifies

This service is free to those who meet eligibility requirements. To qualify for Service Coordination, you must have an intellectual or developmental disability and be determined eligible by the Missouri Department of Mental Health Division of Developmental Disabilities.

The eligibility requirements include:

  1. A mental and/or a physical disability that began before the age of 22 and;
  2. Likely to continue indefinitely and result in functional limitations in two or more areas of life.
