Admission Criteria
Must have a developmental disability or other identifiable disability
and reside within Pettis and Saline counties OR Eligibility
determination completed by the Department of Mental Health, Division of
MR/DD.
Eligibility approval of the CHS Admissions/Placement/Suspension Committee.
Services Provided
- Connecting and building relationships with program participants,
families and parent/guardians.
- Assessment of strengths and needs
- Planning for services and supports.
- Advocacy (IEP process, guardianship, etc.)
- Seek and develop natural supports within the community.
- Referrals to other agencies, services.
- Family Support and Respite for those who qualify.
Persons Served
Any individual receiving services through CHS who reside in Pettis and
Saline Counties (Individuals who reside in other counties will receive
Service Coordination - Case Management from the Regional Center)
Children after age three and adults with a developmental disability or other
identifiable disability.

Referrals & Fees
Referrals for children or adults to CHS may be
made by:
- Individuals
- School district officials
- State agency representatives
- Physicians
- Other interested parties

(Pettis County) or calling 660-886-4261 (Saline County)
The Center operates all programs on a cost-for-service
basis. A combination of third-party payment, direct
payments, or donated funds are available depending on the
individual served and the service rendered.
