Admission Criteria
- Age 18 or older (16 or older for Activity Center)
- Developmental or other identified cognitive and/or physical disability
- Medically and Behaviorally stable with supports available.
- Funding source identified
- Approval of Admissions/Placement/Suspension Committee (APS)
Transition/Exit Criteria
Transition and Exit Criteria for each individual participant will be determined by the individual and his/her Individual Plan team.The team will consist of the individual, his/her Consumer Advocate, Program Supervisor, Coordinator or Program Specialist, and the parent/guardian of the participant, as well as the Community RN, if appropriate and anyone else the individual wishes to include on his/her team.
