Community Integration
Community Support Services
The Community Support Coordinator Program (CSC) provides a
variety of support services and training to assist individuals to live
independently. These services are available in Pettis and Saline
counties. The participant and their interdisciplinary team develop an
individual plan, set goals, and determine what types of training and/or
supports are needed. The CSC then provides the individual with the
training and/or support to help the individual achieve their goals.
CSC’s provide training and support in such areas as cooking, budgeting,
grocery shopping, cleaning, hygiene, social/interpersonal skills, as
well as medication management and scheduling of appointments. |
Day Habilitation Services
Day Habilitation Services are provided in Adult Day Activity
Centers. This program provides individuals with opportunities for
learning, social interaction and community involvement. Available in
Pettis, Saline and Moniteau counties, Activity Center participants may
learn to improve their self-care skills, socialization and community
skills, while also learning to utilize community resources.
