Community Support Coordinator – Part Time I – Less than 19 hours weekly

SUMMARY: As part of an interdisciplinary team, participates in the design, development and implementation of individual plans for participants with developmental disabilities.  Provides skills training, guidance and support to facilitate greater independence, attainment of personal goals and successful participation in the community.  Manages caseload and coordinates with other providers to ensure consistent delivery of quality services.

QUALIFICATIONS: Two year degree in Human Services or related area, or 2 years of related experience.

Program Assistants

  • Part time-29.5 hours weekly-Sun, Wed, Thurs, Fri -3:00pm-9:00pm, Sat-10:00am-3:30pm  High School Diploma or GED
  • Full time-Lead Support Staff-40 hours weekly-Mon-Fri 6:00am-2:00pm. Associates Degree + 1 yr experience or combination of both to equal 3 yrs working with persons having developmental disabilities.
  • Full time-Support Specialist-40 hours weekly-Wed-Sun 2:30pm-10:30pm. High School Diploma or GED; 1 yr exp working with individuals with unique personality traits who may exhibit challenging behaviors or related education in Human Services field; or equivalent combination of education and experience.

  • Part time-28 hours weekly-Sat 10:00am-9:30pm; Sun 10:00am-8:30pm; Mon 3:00pm-9:00pm High School Diploma or GED
  • Part time-17 hours weekly-Support Specialist-Sat & Sun 6:00am-2:30pm. High School Diploma or GED; 1 yr exp working with individuals with unique personality traits who may exhibit challenging behaviors or related education in Human Services field; or equivalent combination of education and experience.

SUMMARY: Responsible for providing instruction and support to persons with disabilities in a home setting for the purpose of achieving personal goals and a greater level of independence in the community.  Provides instruction and assists where necessary with all aspects of independent living, community access, self-care, socialization and health and safety skills.  Promotes self-growth through the implementation of an Individual Planning process.

QUALIFICATIONS: Must be 21 years of age

Job Coach – Recycle Center

WORKING HOURS: Monday- Friday-7:30am-4:00pm

SUMMARY: Trains clients to perform tasks required of a given job Trains clients to operate and/or maintain equipment. Monitors and assists clients with individual work habits and attitudes, interpersonal skills and work performance behaviors. Participates in and directs daily production at the work area/site. Maintains production and quality control records Operates equipment for different recyclable items. Operates forklift and Swap Truck – will certify, clean driving record required

QUALIFICATIONS: High School Diploma or GED

Basic Computer experience preferred

Ability to identify and solve problems in a timely manner

Ability to maintain and complete accurate documentation according to policy and procedure

Minimum 6 months production experience required.  Experience in vocational rehabilitation and/or training is desirable.

Home Visitor III

STARTING SALARY:  $12.91-$17.30 per hour.

SUMMARY:  On a daily basis works with EHS families in collaboration with the Department of Social Services-Children’s Division and also provide opportunities to enrolled Early Head Start families for their personal growth and development in order to attain a greater degree of self-sufficiency and independence.

QUALIFICATIONS:  Bachelor’s Degree in related field and two years of related work experience with infants, toddlers and families and early childhood education is required.

Ability to build relationships with families and children

Some evening work required

Excellent communication, time-management and organizational skills

Must be willing to obtain and maintain a Class E Chauffeur’s License

SUMMARY: Actively and efficiently trains individuals according to the individual plan as assigned using special training groups as needed.  Completes pertinent documentation as directed in a timely manner, including Individual Plan monitoring, incident reports, daily notes on participants, 30 day reviews and quarterly reviews.  Ability to provide transportation and perform personal care for consumers.

Registered Nurse Instructor – Part Time I – Less than 19 hours weekly

STARTING SALARY: $10.72-$16.85 per hour.

SUMMARY: On an as needed basis, works in the Human Resources Department by performing the following duties: provide TB tests and HBV shots as required and develop and conduct training.

QUALIFICATIONS: Must be a Registered Nurse with total education and experience to equal nine years.

Minimum of 3 college credit hours in Early Childhood Education; Child Development Associates preferred

Ability to work well with others; good communication skills are essential

Must be willing to move classroom to classroom as needed to maintain ratios due to staff breaks, planning or staff shortage.

Consumer Advocacy Services Professional – Clay County – Four Positions – Exempt

STARTING SALARY: $15.21-$22.11 per hour.

SUMMARY: On a daily basis, provides professional consumer advocacy services to eligible adults and their families. Assists individuals and families by accessing community resources, conducting both formal and informal assessments, completing paperwork, and securing supplementary information to complete the individual plan for person served.

QUALIFICATIONS: Bachelor’s degree (BA or BS) in related field from an accredited university plus three years of related work experience or Master’s degree (MA or MS) in related field from an accredited university plus one years of related work experience. Experience working with individuals with developmental disabilities in community-based services is required.

Proficiency in using a computer

Ability to schedule appointments and maintain personal calendar

Ability to complete necessary documentation within set guidelines

Excellent interpersonal and communication skills
